THIS IS A GRANT FUNDED POSITION AND MAY BE LIMITED IN DURATIONGENERAL DESCRIPTION OF POSITIONThis is full-time Program/Project Coordinator position in the Outreach department of North Country HealthCare.
The Program/Project Coordinator will implement, develop and support programs associated with the State Opioid Response(SOR) initiative.
The initiative is not limited to care coordination activities for individuals identified to have an opioid use disorder.
The position requires strong organizational and communication skills.
ESSENTIAL DUTIES AND RESPONSIBILITIESSupport and assist in the development of opioid misuse response efforts for North Country clinics in Mohave County.
Train and support clinic staff on the North Country HealthCare pain management protocol.
Support and develop relationships with partners in the community, not limited to behavioral health homes, criminal justice entities and county health departments.
Fulfill the scope of work as dictated by Health Choice Integrated Care.
Travel throughout initiative service area (Coconino, Yavapai, and Mohave Counties) as necessary.
Serve as the main point of contact for the SOR Northern Region Care Coordination Initiative.
Collaborate with HCIC to identify and address individuals identified with an opioid use disorder or as having concerns requiring attention due to opioid use.
Attend and engage in SOR committee meetings and committees related to the initiative.
Coordinate North Country participation in tele-health or tele-mentoring programs sponsored by HCIC, including ECHO.
Complete North Country reporting on activities and patient demographics related to the SOR Northern Region Care Coordination Initiative.
Collaborate with HCIC and NCHC to identify and provide care coordination efforts with partnering agencies for the individualized recovery needs of individuals identified with an opioid use disorder, or symptoms of opioid use disorder, or otherwise identified by HCIC as having concerns requiring attention due to opioid use.
Facilitate, organize, and develop activities for the SOR Northern Region Care Coordination Initiative along with the pain management protocol at North Country.
Develop and coordinate care for a registry of North Country patients requiring MAT and other community supportive services.
Proactively engage with the patient and community partners to achieve improved health outcomes.
Provides coordination and continuity in outreach and navigation efforts across assigned region.
Participate in the planning, implementation and evaluation of the grant work plan.
Train and assist clinic staff and volunteers regarding the SOR initiative, the pain management protocol, as well as North Country and community resources.
Manage the grant budgets and deliverables.
Regular and predictable attendance.

Consistent review, comprehension and reply of company email and related correspondence.
REQUIRED CERTIFICATES, LICENSES, REGISTRATIONSBachelor's degree required.
One or more years of experience in Health, Education, or Social Work fields helpful.
Must have excellent organizational skills.
Skilled in verbal and written communications.
Strong data analysis, and computer skills.
Must be able to work independently, and manage multiple priorities.
Cultural competence with Native American and Hispanic cultures.
Spanish language competency preferred.
Some travel required.
SALARY$40,750.
00/YR Plus depending on qualifications.
