RN Case Manager
Victory Medical Solutions LLC is seeking dynamic RN Care Managers to work in Bullhead City, AZ. This is paid at $30-$35hr D.O.E
Care Managers are in the home as advocates and coordinators to help Members access benefits and resources and as coaches to assist with disease management and self-care. Our goal is to prevent unnecessary hospitalizations and ER visits through three types of programs Brief Assessment, 30-day Transitions and High Severity:
oA Brief Assessment is a one-time visit that has been requested to evaluate the member using the member survey, validate the areas of focus and provide recommendations.
oThe 30-day Transitions program is for Members that are recently discharged from a hospital or rehab facility. A minimum of 3 visits are required to provide extra support.
oThe High Severity refers to members that have been referred to receive weekly face to face visits in the Long Term In-Home Program.
Requirements: RN with at least 2-3 years of home health experience, must pass nurse/social competencies and backround check, excellent communication & computer skills, must be dependable and efficient with electronic charting (turn-a-round within 24 hours).
