The purpose of the M.S.W. - Admission / Discharge Coordinator position is to provide administrative support to the Acute Inpatient Rehab Unit by coordinating the patient admission and discharge process in order to promote a smooth operation of the rehab department, as required by Federal Regulations, the State of Arizona and JCAHO. This includes but is not limited to Social Services and Case Management. This position works closely with The Rehab Medical Director, Program Director, Clinical Manager, Patients, Families, and the Rehab Nursing and Therapy Team, Physicians and other professionals to coordinate admissions through the Rehab prescreening process, including verifying insurance and setting up transportation, and timely entry of patient information into the Electronic Medical Record and IRF Software per CMS guidelines. This position facilitates weekly team meetings and family conferences. This position provides guidance, support and resources throughout the discharge process, coordinating home needs, medications, outpatient therapy services and assists with the integration of the patient back into the community. This position helps the patient and family understand the rehabilitation process and offers emotional, social, and financial counseling while the patient is in the rehab hospital. They will assist with patient and family education, and identify specific community social and health care resources to facilitate a patient in returning home. Other duties may include downloading and transmitting patient data to CMS per regulatory guidelines, adjustment counseling, mental status testing and other administrative support/duties as needed or assigned.

ACCOUNTABILITY:
The Admission / Discharge Coordinator is accountable to the Rehab Medical Director Program Director, Clinical Manager and ultimately accountable to the Chief Nursing Officer.

SUPERVISORY RESPONSIBILITY:
No supervisory responsibility.

CONTACTS/INTERACTS WITH:
Patients & Families, Employees, Physicians, other professionals and community member

QUALIFICATIONS
Degree / Licensure / Certification / Registration: Master of Social Work
Experience Required: Professional experience in Medical Social Work and/or in the Rehab setting.

Must possess the ability to organize tasks among team members. Must have strong interpersonal, oral and written communication skills. Effective human relations skills are required for interfacing with team members, all levels of staff, physicians, patients, families and other contacts. Must possess the ability to effectively function in a stressful environment. Must possess the ability to use the following equipment: Telephone / Overhead Paging System / Fax Machine/ Calculator / Computer
Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
